Clarest Health Strengthens Institutional Pharmacy Position with Remedi SeniorCare Acquisition

Clarest Health, a leading personalized medication management provider, announced that it has acquired Remedi SeniorCare, a long-term and post-acute care pharmacy services provider, giving the company the size and scale to lead the post-acute care market. ProCare LTC, a leading provider of professional solutions for post-acute pharmaceutical care, rebranded as Clarest Health in 2022.

This acquisition represents another stake in Clarest Health’s strategy to provide flexible medication management and reduce the complexities that patients and their caregivers face, as patients transition to and from the hospital, skilled nursing facility and home environments.

In September 2022, Clarest Health purchased CSS Health whose proprietary technology offers real-time insights to identify vulnerable patients to ensure medication adherence. In October 2022, it purchased Integrity Pharmacy, a “pharmacy at home” service to serve patients in their home environment. Now, with the acquisition of Remedi, Clarest Health expands its geographic footprint and technological capabilities.
