AccentCare and Seasons Hospice Merger to Reshape Home Care and Hospice Market

AccentCare, owned by Advent International, is among the five largest home health providers in the country. Seasons Hospice holds the same distinction in the U.S. hospice market. The merger will expand the combined company’s geographic footprint and portfolio of services.

AccentCare delivers home health, hospice, personal care and care management services to more than 145,000 patients and clients annually, doing so across more than 179 locations in 17 states.

Seasons Hospice & Palliative Care offers end-of-life care services to upwards of 30,000 patients a year. Its current operations span 19 states and 31 Medicare-certified programs, inclusive of 22 facility-based and freestanding in-patient centers.

The combined AccentCare-Seasons enterprise will operate over 225 sites of care across 26 states, employing nearly 30,000 workers. It will have over 60 total partnerships with health systems and physician practices, collectively providing care to more than 175,000 patients and families each year.

In addition to Fairview Health Services, AccentCare’s existing joint ventures include partnerships with Asante, Baylor Scott & White Health, UCLA Health and several others. Seasons Hospice has more than 30 strategic partnerships.

In late 2019, both AccentCare and Seasons Hospice initiated pharmacy-based activities:

  • AccentCare selected Hospice Pharmacy Solutions (HPS) to provide pharmacy benefits management (PBM) services to hospice patients nationwide. HPS provides AccentCare patients who receive home-based care or reside in a facility with same-day and next-day delivery of prescription medications through a national network of community-based pharmacies. Where needed, mail order delivery will also be utilized in certain areas of the country.
  • Four Seasons launched a pilot telehealth-based pharmacy program that will include a virtual pharmacist system for patients who live in rural areas of their home state. Four Seasons will be collaborating with pharmacy benefit manager Delta Care Rx on the project.
